You have arrived at the Registration Form for OMOS and HC Worship Sites! If you are in need of Sacramental Records or Godparent/Confirmation Sponsor Letters, please go to the Records Request page under the SACRAMENTS Tab. OCIA Registrants should go to GET INVOLVED/OCIA page on the Main Menu Bar.
You can register two adults in the same household as well as up to three children. There are several general questions for the household. When you have entered all of your data, you can scroll down to the last couple of questions, click on the Spam and Bot checks, then click the Submit Button right below the Bot Check and the form will be sent.
There is a space near the end of the form for you to send any comments or questions you might have for the Office before you send it.
The form, when successfully submitted, goes directly to the Office In Box at Our Mother of Sorrows or Holy Cross Parishes. The staff will have your Email Address and/or Phone(s) in case there are questions. There will be a brief message confirming receipt appearing at the bottom of this page once the completed form is received in the Office. This Green Colored Message should appear in just a few seconds after you SUBMIT.
Some information is mandatory (there is a small asterisk near the entry field) and if not completed, the system will take you back to the field in need of attention before it will let you submit the form. Once filled in properly, you can scroll down and SUBMIT the form!
