The form, when submitted, goes directly to the Office In Box at Our Mother of Sorrows or Holy Cross Parishes. The staff will have your Email Address and/or Phone(s) in case there are questions. There will be a brief message confirming receipt appearing at the bottom of this page once the completed form is received.
